Aims

Evaluation of epidemiological data of Alzheimer's disease, multi-infarct dementia and Parkinson-dementia in the longitudinal study population.
Evaluation of the efficacy of brain imaging studies (PET and SPECT) in the clinical differential diagnosis of the dementing syndromes.

Scientific activity

Epidemiological study - neurologic evaluation.

Epidemiologic and neurological evaluation has been performed in 423 subjects (94 within the protocol for dementia, 52 within the protocol for parkinsonian syndromes, 144 within the protocol for symmetric polineuropathy, 62 within the protocol for cerebrovascular diseases).

Functional neuroimaging in dementia and Parkinson's disease.

We used SPECT with perfusion (HM-PAO, ECD) and D2 receptors (IBZM) tracers and, in some patients, PET with 18F-fluorodeoxyglucose. In dementia, neuroimaging studies addressed two topics: 1- evaluation of SPECT and PET findings in terms of diagnostic specificity and sensibility, 2- D2 receptors study in DAT in relation to extrapyramidal symptoms. Striatal D2 receptors imaging in Parkinson's disease has been evaluated in relation to: 1- therapeutic response to L-Dopa in idiopathic Parkinson's disease, 2- differential diagnosis of idiopathic Parkinson's disease and parkinsonisms (in this case, together with cortical metabolic and perfusion findings).

Scientific results

We performed SPECT with a perfusion tracer and a D2 receptor ligand in Alzheimer's dementia patients and in patients with a clinical diagnosis of frontal lobe dementia. Whereas HM-PAO SPECT could not separate the two groups of patients, IBZM SPECT showed a significantly reduced uptake in frontal regions of the group of patients with frontal lobe degeneration.

IBZM SPECT was used to determine D2 receptors number in basal ganglia regions of Alzheimer patients without overt extrapyramidal symptomatology. Caudate and putamen D2 receptors number was significantly reduced as compared to age-matched control subjects.

In Parkinson's dissease patients, striatal D2 receptors number was determined in groups of patients separated on the basis of their response to therapy. Significantly reduced IBZM striatal uptake was found only in the group of patients with a fluctuating response to levodopa.
